Subungual Hematoma

Blood pooling under the toenail -- painful and treatable.

A subungual hematoma is a collection of blood beneath a toenail, usually caused by trauma (dropping something on the toe, stubbing it, or ill-fitting shoes during running). The pressure from the blood causes intense throbbing pain.

Treatment Approach

Dr. Husein's philosophy

80% of cases resolve with conservative care

Conservative Care First (Preferred)

  • Decompression (trephination) -- Dr. Husein drills a small hole to release pressure
  • Elevation and ice
  • Anti-inflammatory medications
  • Nail protection padding
  • Monitoring for infection

Surgical Consideration (Last Resort)

Surgery is always the last resort.
Removal of the nail may be necessary if the nail is severely damaged or if the underlying nail bed requires repair.
